If you engage SwiftWorks Solutions LLC for documentation services, that relationship is governed by a separate written service agreement and, where protected health information is involved, a Business Associate Agreement. Where those documents conflict with these terms, those documents control.

SwiftWorks Solutions LLC is a United States–registered company providing physician-led medical transcription and clinical documentation support to healthcare providers and organisations.

03Acceptable use

You agree not to:

• Use the site for any unlawful purpose.
• Attempt to gain unauthorised access to the site, its servers, or any connected system.
• Interfere with the operation of the site, including by submitting automated or bulk form submissions.
• Submit false information, or impersonate another person or organisation.
• Copy, scrape, or republish the content of this site without our permission.

04Do not submit patient information

This website is not a secure channel for protected health information. You must not submit patient names, identifiers, clinical details, or any other identifiable patient information through the enquiry form or by email to us.

If you do, we will delete it rather than retain or forward it, and we will ask you to use an appropriate secure channel. You are responsible for ensuring that anything you send us through this site is free of patient information.

05No professional or clinical advice

The content of this site is provided for general information about our services. It is not clinical advice, legal advice, coding advice, or compliance advice, and it should not be relied on as a substitute for professional judgement.

Any sample clinical documentation shown on this site — including the example SOAP note — is a fictional composite created to demonstrate structure and formatting. It does not describe a real patient, is not drawn from any real encounter, and contains no patient information of any kind.

07This site is not an offer

Descriptions of services, engagement models and turnaround on this site are indicative. They are an invitation to discuss an engagement, not a binding offer, and they do not create a client relationship or a business associate relationship.

Pricing is quoted per engagement. Any figure discussed on this site or in correspondence is subject to a written agreement before work begins.

The content, design, text, graphics and layout of this site are owned by SwiftWorks Solutions LLC or used with permission, and are protected by applicable intellectual property laws. The SwiftWorks name and logo are our marks.
